This is the form used to add, amend or delete names on the electoral register under rolling registration.

To register to vote, or to update your details, please return your completed form to:

• Electoral Services
  Room 26, Lambeth Town Hall
  Brixton Hill
  London SW2 1RW.

Please note: you cannot register online to vote.

• If you are unable to download this form please contact our Electoral Services team - they can send a form to you.
• Only one person can apply per form. You must sign your own form - someone else cannot do it for you.

The edited register

If you do tick the box on the voter registration form for your name to be excluded from the edited register, your name and address will only appear on the full register. Information from the full register may only be used for certain lawful purposes, such as elections, law enforcement and checking applications for credit.

If you do not tick this box, your name will also appear on the edited version of the register, which anyone can buy. Please see the electoral register page for further information.

Postal and proxy votes

• If you require a postal vote, please complete and return the postal vote application form.
• If you need to vote by proxy please contact the Electoral Services team using the details below. They will send you the relevant form for completion.
