Leaving hospital

If you are going into hospital you may need help when you leave to continue to live in your home.

If requested, a social worker from adults' and community services will look into what help you need and decide with you what services can be provided.

You can either contact adults' and community services before you go into hospital or you can ask to see a social worker during your stay in hospital.

The nurse looking after you will arrange this if you ask.

Support for older people leaving hospital

Lambeth adults' and community services has hospital social work teams based in St. Thomas' Hospital and Kings College Hospital, who can help you to plan your care on leaving hospital.

If you are in hospital and you feel that you need the services that a hospital social worker can provide, you can contact the hospital social work services at St Thomas' and at Kings College hospitals.

The hospital staff will contact adults' and community services for you, so please speak to a nurse or other health professional involved in your care.

If you prefer, you can ask a friend or relative to speak to them for you.
