Southwark and Lambeth Integrated Care Pathway for Older People with Falls (SLIPS)
Southwark and Lambeth councils have worked together to develop an integrated service for people who have suffered a fall, or are just worried that they might be at risk of falling. The service is called SLIPS (Southwark and Lambeth Integrated Care Pathway for Older People with Falls) and brings together the NHS and the social care, voluntary and leisure sectors in Southwark and Lambeth. The SLIPS service means that patients now get the same assessment and the same menu of treatments no matter which falls clinic they attend in Lambeth and Southwark.
Although you can be referred to the service by your GP or social worker/care manager, if you've had a fall in the past year, you can also refer yourself. Just complete theĀ basic falls assessment form on the SLIPS website, which will guide you to the help you need.
The SLIPS provides advice on rehabilitation and preventative strategies for those who have fallen and/or are at risk of falling, in order to reduce the number of people having falls throughout both boroughs.
