If you have a change in your or one of your household’s medical circumstances, please complete a Medical Assessment form and a Change of Circumstances form.

If you need to move because of health or the disabilities of someone you list on the form. You must complete both the Housing Register form and the Medical Assessment form. You must complete one medical form for each person that has a medical condition.
