Hospice care

Hospices provide care specifically for terminally ill people and give support to their carers both in the hospice and at home.

Residential care

A residential care home provides accommodation, meals and personal care for older people, people with disabilities, or people who are unable to manage at home, for whatever reason.

Sheltered housing

We assess people who are interested in being considered for sheltered housing. We manage sheltered housing schemes across Lambeth, designed to encourage and support independent living in the community.

Supported Needs Assessment and Placement (SNAP) team

The Supported Needs Assessment and Placement (SNAP) team is the first port of call for those requiring housing related support services. The team is funded by Lambeth's supporting people services.

Supporting people - housing support services

The Supporting People programme funds housing support services in Lambeth, so that vulnerable people are given the opportunity to improve their quality of life by enabling them to live independently in the community.

Supporting People - Regional Champions Resource

The Lambeth Supporting People Team are Regional Champions for governance. This is a resource for Supporting People teams and Local Strategic Partnership (LSP) colleagues to promote what we have done and share our approach.
