Continuing Care

Some children and young people have long-term health needs arising from disability, accident, or illness. The continuing care team provide care packages and support post-discharge including signposting, respite, equipment and advice.

Children and Young People (0 to 18 years) living in and registered with a GP in Lambeth and Southwark who meet the eligibility criteria can access the service.

Any child or young person can be referred to the service by a healthcare professional providing they meet the eligibility criteria. Please note families or carers cannot self-refer.
