Before contacting the Lambeth Information, Contact and Assessment Service please ensure you have:
- asked yourself if the Information, Contact and Assessment Service need to know this information- attempted to resolve the issue which has arisen yourself- spoken to the person you are caring for - for example you have spoken to the service user and have discussed and agreed any time changes of care before telling Adult Social Care- attempted to contact the service user’s family if they can assist.
If the purpose of contact is concerning a health issue, please ensure you have made contact with the service user’s GP or district nurse.
If you need to do this, fill in the safeguarding adults form.
If your referral or concern is urgent and you would prefer to speak with someone, call 020 7926 5555 and ask to speak to the Information, Contact and Assessment Service.
To provide services for you, we ask for a wide range of information, some of which is personal and may be required by law.
