Health passport

Many young people leaving care feel frustrated at their lack of knowledge around their own health and medical history. This is why the health passport has been developed.

Your health passport is a quick reference guide to confidential details about your personal health. It is for you to keep and update at any time.

You can add information about your doctor, dentist, optician or any other medical professional, detailing everything from medical conditions you may have to when you last visited your orthodontist.

After your last health assessment, when you are 18 years old, your nurse will provide you with your health passport.

Give a copy of your health passport to your doctor when you register with one. It will help them to understand your medical history and needs.
