Pathway plan

You will complete a pathway plan with your social worker or personal advisor between the ages of 18 and 21, or older if you stay in education.

The pathway plan is your plan and in it you will set out your needs, views and future goals.

Your pathway plan will also look at your education, training, health, money, relationships, where you are living and will be living, and where you feel you may need support with these areas to reach your full potential.
