Short breaks providers

For the 0-25 Disabilities Service, places at all providers must be arranged via the short breaks co-ordinator. Each referral is subject to an assessment to ensure that the project can meet individual needs.

Lambeth's commissioning team for short breaks and home care wants to hear from parents, guardians and carers of children, in order to improve the current and future services we offer.

Give us your feedback on short breaks and home care

If you do not wish to complete this form but want to comment on our short breaks, home care and/or continuing care services, please email us at childrenssendandhealth@lambeth.gov.uk.

Notification of Absence

Parents and Carers are expected to provide at least 48 hours’ notice if their child or young person will be unable to attend a booked session at an Afterschool Club, Saturday Club, or Holiday Scheme. Please email your Social Worker, Short Breaks provider and the Short Breaks Co-ordinator at ShortBreaksCoordinator@lambeth.gov.uk in cases of planned absences, such as hospital appointments, family visits, or holidays.

If an unexpected situation arises, such as illness, please inform the Short Breaks provider and the Short Breaks Co-ordinator as soon as possible.

Repeated failure to provide 48 hours’ notice for non-attendance may result in a reduction or termination of your Short Breaks package.
