As you get older, your care will move from children’s health services to adult health services. This change is called transition and will happen after your 18th birthday.
As an adult, you may still need support. Different adult health services are available, and who helps you will depend on your health needs and the care you currently get.
Click on the tiles below to learn more about the transition process that matches your health needs and current care.
