Team 4 will work with the young person and review the Care and Support plan every 12 months at least.
The young person will not always have a named social worker, but they will be given a social worker when the Care and Support plan needs to be reviewed.
The young person will usually stay in Team 4 until they have finished education and the Care and Support plan is still meeting needs, or until they are 25 years old.
What happens next?
The young person will move from Team 4 to the Adult Social Care Team that is right for their needs. This is usually the Adult Learning Disabilities Team or the Adult Physical Disabilities Team.
The new team will make contact when the Care and Support plan needs to be reviewed. Contact them if you think it needs to be done sooner.
A social worker in Team 4 will talk to you both about changing teams closer to the time so you know what happens next and who to contact in the Adult Social Care Team.
