This information is about young people who are not currently supported by the 0–25 Disabilities Service
We support children and young people who:
- Are between 0 and 17 years old
- Live in Lambeth
- Have serious and long-term difficulties or health problems
How to get support
If you are a parent or carer, call 020 7926 5555 or email helpandprotection@lambeth.gov.uk
If you are as professional, call 020 7926 310 or complete a Multi-Agency Referral Form (MARF).
What happens next?
- Someone from the Initial Referral Hub reviews the referral and decides if an assessment is needed
- If an assessment is needed, they will decide which team will do this
- If your child has serious and long-term difficulties or health problems, the 0-25 Disabilities Service would do a Child and Family assessment.
Who can get specialist help?
To qualify, children and young people must:
- Be 0 to 17 years old
- Live in Lambeth
Have one or more of the following:
- A serious or permanent disability
- More than one disability, including mental health issues
- Autism with challenging behavior
- Severe hearing or vision loss
- Mobility problems needing a wheelchair or special equipment
- Serious health problems like epilepsy or a life-limiting illness
- Needs that cannot be met by universal short breaks
- A parent or carer who needs overnight breaks
What is a child and family assessment?
A social worker will:
- Talk with the child. family and others involved
- Ask for medical or other important documents
- Figure out what kind of support is needed
What support can be offered?
- Advice and information
- Direct Payments - money to arrange your own care.
- Help at home
- Specialist Short Breaks activities
- Health-related support
How we review support:
- Short Breaks - reviewed every 6 months
- Child in Need - visits every 3 weeks. meetings every 12 weeks
- Child Protection - visits every 10 working days
- Looked After Child - weekly visits at first. then every 4 weeks
