14 to 17 years old supported by the 0–25 Disabilities Service

At age 14 the social worker and school will start talking to you about Preparing for Adulthood.  This may be done at annual reviews at school. It can also happen at other meetings. 

We talk about: 

  • Education or work
  • Money and benefits
  • Living independently
  • Health care after age 18 

At age 17, the Children’s Services social worker refers you to Adult’s Services.  Children’s and Adult Social Care will work together with you both to plan transition to adulthood. 

Before your child’s 18th birthday, a social worker from Adult Social Care will complete a Care Act assessment with you both, then create a Care and Support plan. 

Adult Social Care will continue to work with you both following the 18th birthday and review the Care and Support plan every 12 months at least. 

In Adult Social Care a young person may need to pay money towards the care package. We will carry out a financial assessment. Find out more here.
